Please confirm that your provider is in-network with your insurance company prior to your first appointment.

Fees for insurance networks are pre-contracted.  The client/patient is responsible for all co-pays, co-insurance, and/or any fees not covered by insurance.  Due to insurance limitations, some of our providers do not accept insurance reimbursement.  However, we can provide for you a statement to be submitted to your insurance company.

If we are a participating provider with your insurance plan or have an agreement with your insurance company, we will submit insurance claims for you.  You will need to find out the details of your insurance policy for out-patient mental health, including deductibles, co-pays, co-insurances, pre-authorizations, physician referrals, and any other limits.  If we do not participate with your insurance plan, we will provide for you a statement that can be submitted to your insurance company by you.  It is your responsibility for any outstanding balance not covered by insurance and agree to pay any such amounts in a timely manner. 

We currently do not participate with Medicare, Medical Assistance, or Medicaid.  Including, but not limited to, Priority Health Partner's.
